10 most common sentinel events in Q2 2016

The Joint Commission received 439 sentinel event incident reports in the second quarter of 2016, which the agency notes only represents a small fraction of actual events.

Here are the 10 most commonly reported sentinel events in Q2:

●    Unassigned (Category unassigned at the time of report) — 72
●    Unintended retention of a foreign body — 52
●    Fall — 52
●    Wrong-patient, wrong-site, wrong-procedure — 47
●    Suicide — 44
●    Delay in treatment — 28
●    Op/post op complication — 23
●    Other unanticipated event  (including asphyxiation, burn, choked on food, drowned or found unresponsive)  — 19
●    Criminal events — 18
●    Medication error — 14
