3 ASCs utilizing patient navigation tools

Here are three ambulatory surgery centers that are using patient navigation tools.

Orthopaedic South Surgical Center (Morrow, Ga.). The surgery center uses One Medical Passport, an internet-based pre-assessment system.

"Using the One Medical Passport online pre-admission system, our patients can enter information, save it and pick it up again at a later time. All data previously entered can be updated if information changes and used for subsequent procedures. Patients can print copies, read preop instructions and disclosures online — everything is in one place," says Andrea Fann, CASC, administrator of the facility. "Because the information we receive is more accurate and thorough, our nurses are spending less time asking questions and more time focusing on patient care, including instructions and answering questions. Greater accuracy also helps with medication reconciliation and reduces delays or cancellations. This patient navigation tool is a favorite among our staff and patients alike."

The surgery center is accredited by the Accreditation Association for Ambulatory Health Care, licensed by the state of Georgia and Medicare-certified. It is an affiliate of United Surgical Partners International and partnered with local physicians. The facility includes three operating rooms and one treatment room, and offers services in orthopedics and pain management.

UW Hospital and Clinic's outpatient surgery center (Madison, Wis.). UW Health is using Wellbe's patient navigation software for ACL reconstruction surgeries at its outpatient surgery center, and will be using it at UW Health at The American Center, a new hospital, outpatient care and wellness facility in Madison, opening in August, 2015.

Wellbe's cloud-based platform combines tools for patient engagement and care coordination, and also provides real-time insight from patient-generated data.

"The program empowers patients and their families to be as prepared and engaged as possible across the entire continuum of their care, from initial assessments all the way through treatment, rehabilitation and recovery," said Kip Schick, director of orthopedics for UW Health. "By increasing patient engagement from the start, we hope the program will further improve patient confidence and, ultimately, their outcomes."

The UW Hospital and Clinics Outpatient Surgery Center, which has provided care for more than 10 years, has an annual caseload of more than 9,500 outpatient operative procedures.

Gulf Pointe Surgery Center (Port Charlotte, Fla.). Gulf Pointe Surgery Center has implemented the One Medical Passport, an online patient pre-assessment system.

"Better information makes for better processes and a positive patient experience," says Alix Hall, administrator of the center. "Since bringing our pre-admission process online with One Medical Passport and allowing our patients to take control of their medical histories, information is more complete, organized, consistent and reusable. Because information is submitted by the patient and reviewed by nursing and anesthesia well in advance of a procedure, our patients arrive prepared and confident that they are in professional hands. Using the patient navigation tool there is no missing documentation, no morning-of scramble, no repetition and no surprises. The result is a calm and orderly process."

The surgery center offers ophthalmology, general surgery, GI/endosopy, orthopedics, podiatry, ENT, urology, gynecology, neurology and pain management services. It is Medicare-certified, accredited by the Association for Ambulatory Health Care and licensed by the state of Florida.
