Tri-Cities Gastroenterology, a GI practice with five locations in Tennessee, has suffered a data breach, according to an April 30 report from Claim Depot.
The breach occurred on or about Dec. 11, 2025, when an unauthorized third-party removed files from the practice’s network.
Information that was accessed by the unauthorized party includes full names, Social Security numbers, dates of birth, addresses, email addresses, telephone numbers, gender, and medical record numbers, according to a notice from Tri-Cities Gastroenterology.
Tri-Cities Gastroenterology began notifying individuals and disclosed the breach to the Vermont Attorney General on April 29, the report said.
