Mannitol Plus Simethicone Provides Improved Bowel Prep Before Capsule Endoscopy

Research findings suggest split-dose oral mannitol plus single-dose simethicone provides better small bowel preparation before capsule endoscopy than other bowel preparations, according to a study published in the Journal of Clinical Gastroenterology.

Two hundred patients were divided into four groups. The first group had a clear-liquid diet after lunch on the day before their capsule endoscopy, followed by overnight fasting. The second group took 250 mL 20 percent mannitol and 1 L 0.9 percent saline orally at 5:00 a.m. on the day of the procedure. In the third group, the same regimen was taken at 8:00 p.m. on the day before and at 5:00 a.m. on the day of the procedure. In the fourth group, in addition to the third group’s regimen, 20 mL oral simethicone was taken 30 minutes before capsule endoscopy.

Results showed bowel preparation for the fourth group provided better mucosal visualization for the proximal and distal small bowel. The mannitol and simethicone preparation also provided improved diagnostic yield when compared with overnight fasting.
