•    25 to 49: 23 percent
•    50 to 75: 27 percent
•    76 to 99: 21 percent
•    100 to 124: 12 percent
•    125 to 149: 5 percent
•    150 to 174: 2 percent
•    175 to 200: 1 percent
•    More than 200: 1 percent
•    No patient visits: 2 percent
