Miami-based Gastro Health, a GI practice with a network of 207 locations, has provided notice of a data security incident.

On Feb. 25, the company learned that some of its employees had responded to a phishing incident, which led to unauthorized access to certain files. On March 2, Gastro Health learned of a second phishing incident, which also resulted in unauthorized access, according to a recent notice provided by the organization.

Information that may have been impacted includes names, dates of birth, Social Security numbers, government-issued or state-issued ID numbers, medical record numbers, patient account numbers, Medicare or Medicaid numbers, health insurance or group account number, diagnosis or treatment information, prescription information and provider or clinic information.

According to filing with the U.S. Department of Health and Human Services Office for Civil Rights, more than 37,000 individuals were impacted by the two separate incidents.

Gastro Health does not have any indication that the accessed information has been misused, the notice said.
