Cleveland Clinic Children’s launched a multidisciplinary clinic for pediatric heart transplant recipients focused on long-term health and quality-of-life outcomes.
The program, called Thrive, which stands for transplant, health, resiliency, independence, vitality and empowerment, began in 2025 and provides annual evaluations for patients beginning one year after transplant, according to a May 7 news release.
The clinic brings together specialists in heart transplant cardiology, nephrology, psychology, social work, pharmacy and nutrition during a single visit. Patients also undergo bloodwork, echocardiography and electrocardiogram testing.
The program is designed to identify complications tied to long-term transplant care, including kidney disease, hypertension, medication side effects, mental health concerns and challenges with school or social development.
Staff are also tracking clinical metrics including new diagnoses of chronic kidney disease and hypertension, as well as referrals to other specialties, according to the release.
