Here are five points:
1. The grants would be offered to as many as 21 recipients.
2. Each recipient would receive as much as $200,000 annually for three years.
3. Recipients would use the grant money to enhance health and decrease ER visits associated with chronic disease.
4. Eligible applicants have to offer healthcare in rural areas and must be family physicians, rural health clinics, community health centers or federally qualified health centers.
5. CMS-certified critical-access hospitals and rural health clinics are eligible.
