2. The change would allow the Tullahoma, Tenn.-based practice to add surgical specialties to its existing ambulatory surgical treatment center for an estimated $115,000.

3. The ASTC in question has one operating room and is wholly owned by Jeffrey B. Marvel, MD.

4. Ambulatory Surgical Associates, a recruiting and placement firm, and Tullahoma-based Tennova Healthcare-Harton are opposed to the change.

5. The application is pending state approval.
