5. Gladiolus Surgery Center in Fort Myers, Fla.: April 1
6. Dublin (Ohio) Surgery Center: May 1
7. Thomas Johnson Surgery Center in Frederick, Md.: May 1
8. Grove Place Surgery Center in Vero Beach, Fla.: June 2
9. Naperville (Ill.) Surgical Centre: June 15*
