Patient Safety Tool: Safe Surgery Checklist

The Harvard School of Public Health offers a free, downloadable checklist designed to promote safe surgery in operating rooms.

The checklist was originally created for South Carolina hospitals, where it is currently being piloted with the intention of having the checklist in every U.S. hospital by 2015. Successful implementation and proper use of the checklist is expected to save more than 500 lives per year in South Carolina.

There are two templates to account for the presence or absence of the surgeon in the OR at the time of induction. Download the templates for the safe surgery checklist (DOC) for use in your facility.
