The Agency for Healthcare Research and Quality offers a free toolkit to help hospitals implement a readmission reduction program called Re-Engineered Discharge.
Researchers at the Boston University Medical Center developed RED, which they found was effective in lowering readmissions and post-hospital emergency department visits. The RED is comprised of 12 components, including identifying a need for and obtaining language assistance, making appointments for follow-up care and planning for follow-ups of test or lab results pending at discharge.
AHRQ contracted with BUMC to develop the RED toolkit, which outlines 11 steps to implement the RED and includes multiple resources to support hospitals' adoption of RED.
