Patient Safety Tool: Provider Toolkit to Improve Hypertension Control

The American Medical Group Foundation and American Medical Group Association offer a free toolkit on improving hypertension control among patients.

The "Provider Toolkit to Improve Hypertension Control" is part of AMGF's and AMGA's three-year Measure Up/Pressure Down campaign, which aims for medical groups and health systems to get the blood pressure of 80 percent of their high blood pressure patients in control by 2016.


The toolkit is a guide for healthcare providers to implement a hypertension quality improvement initiative, and is organized around eight care processes. A set of steps and resources, best practice tools and a recommended case study accompany each care process.
