The Agency for Healthcare Research and Quality offers a free, downloadable toolkit designed to help healthcare providers improve medication safety for high-risk Medicare patients.

The 'Improving Medication Safety in High Risk Medicare Beneficiaries Toolkit' is intended to help interested providers adopt an ambulatory-based medication therapy management program. The main components of the MTM model are medication reconciliation, assessment of drug-related problems and resolution of identified drug-related problems. The toolkit includes specific forms and templates to aid in the implementation of the MTM model.
