The Canadian Patient Safety Institute offers a free, downloadable hand hygiene surveillance instrument.

The surveillance tool was developed by researchers at Yale's Department of Epidemiology and Public Health. It allows healthcare organizations to observe and document hand hygiene events and determine opportunities for improvement.
