Aligning Forces for Quality offers free, downloadable discharge planning tools for heart failure patients.

The resources were crafted by Bay Area Hospital in Coos Bay, Ore.

The heart failure discharge planning algorithm is intended to highlight low, moderate and high risk criteria for heart failure patients and then assign discharge orders for each category. The supplemental discharge planning chart sticker is used to facilitate the communication and discharge planning among physicians and nurses.

These tools can be jointly used to reduce readmissions among heart failure patients.
