The Centers for Disease Control and Prevention offers a free toolkit on notifying patients of an infection control lapse or disease transmission.

The toolkit is designed to help healthcare facilities and state and local health departments conduct a patient notification following unsafe injection practices or other errors in basic infection control. The toolkit includes four sections:
