Patient Safety Tool: Antibiotic Regimen Selection for Surgery Pocket Card

IPRO, the Medicare Quality Improvement Organization for New York state, offers a free pocket card for selecting a prophylactic antibiotic regimen for surgery.



The pocket card was created by Stratis Health and the Oklahoma Foundation for Medical Quality and adapted by IPRO. The card is based on recommendations from The Joint Commission's Specifications Manual for discharges from July 1, 2012 through Dec. 31, 2012, and includes approved antibiotics for the following procedures:

•    Coronary artery bypass grafting and other cardiac or vascular procedures
•    Hysterectomy
•    Principal procedure code of hysterectomy with another procedure code of colon surgery
•    Hip/knee arthroplasty
•    Colon
