The card is based on recommendations from The Joint Commission’s Specifications Manual for discharges from July 1, 2012 through Dec. 31, 2012, and includes approved antibiotics for the following procedures:

• Coronary artery bypass grafting and other cardiac or vascular procedures
• Hysterectomy
• Principal procedure code of hysterectomy with another procedure code of colon surgery
• Hip/knee arthroplasty
