The Robert Wood Johnson Foundation's Aligning Forces for Quality initiative offers a free toolkit on engaging patients in quality improvement in primary care.

The "Engaging Patients in Improving Ambulatory Care" toolkit is a compilation of patient engagement strategies in primary care by three of AF4Q's alliances — Maine, Oregon and Humboldt County, Calif. The toolkit includes videos and other resources to help primary care practices recruit, orient and train patients; clarify roles and responsibilities; and establish a structure to foster relationships.
