Patient Safety Tool: AHRQ's Preventing Falls Toolkit

The Agency for Healthcare Research and Quality offers a free preventing falls toolkit to help hospitals implement a fall prevention program.

The toolkit was developed under a contract with AHRQ through the ACTION II program — Accelerating Change and Transformation in Organizations and Networks — and was created by a team including staff from the RAND Corp., ECRI Institute and Boston University.

"Preventing Falls in Hospitals: A Toolkit for Improving Quality of Care," includes an implementation guide, an introductory letter to stakeholders, an action plan, an interdisciplinary team tool and other resources.

Download the preventing falls toolkit for use in your own facility.
