The RARE Campaign — Reducing Avoidable Readmissions Effectively — offers three free tools to review charts and medical records of patients readmitted to the hospital to prevent future readmissions.

The Care Transitions Abstraction Template includes questions for healthcare providers to assess readmitted patients' discharge process and identify possible reasons for readmission.

The chart review tool enables providers to review readmission-related chart information, such as whether a follow-up physician visit was scheduled prior to discharge, for five patients.

The Hospital Readmission Review examines potential readmission factors in nine categories: downstream provider issues, patient/caregiver education, patient activation, medication management, transitional care planning, discharge instructions, end of life, discharge disposition and recurring possible triggers for readmission. This tool was prepared by Quality Insights of Pennsylvania.
