Indiana State Report Reveals Pressure Ulcers Top List of Hospital-Acquired Conditions in 2010

Stage three or four pressure ulcer was the most-reported hospital-acquired condition in 2010 among Indiana hospitals, according to the Indiana State Department of Health's Medical Error Report for 2010.

The annual report is based on the National Quality Forum's 28 serious adverse events. In four out of five years, pressure ulcers have been the most-reported event.

There were 107 reported events in 2010, which is slightly higher than the 105 events reported in 2007 and 2008 and is the most number of reported events in the five-year history of the report. Some of the increase is attributable to a 2009 change in the falls standard that likely resulted in an increased number of reportable falls events.  

The following hospital-acquired conditions were reported in 2010:

•    Thirty-four stage 3 or 4 pressure ulcers acquired after admission to the hospital.
•    Thirty-three foreign objects retained in a patient after surgery, 30 of which occurred in hospitals and three which occurred in ambulatory surgery centers.
•    Seventeen falls resulting in a death or serious disability.
•    Fourteen surgeries performed on the wrong body part, 12 which occurred in hospitals and two which occurred in ambulatory surgery centers.
•    There were no reported medication errors resulting in a death or serious disability.
