Health information for 3,000 clients sent to wrong addresses in Colorado breach

Letters containing personal health information were sent to incorrect addresses due to a computer malfunction in Colorado, according to The Denver Post.

Here are five key points:

1. This is the second breach this year of the Colorado Benefit Management System, which gives aid to residents in need of insurance, food and financial aid.

2. The clients affected received Medicaid, food assistance and financial benefits.

3. More than 1,600 letters contained personal health information and 50 letters contained birthdates.

4. Social security numbers of clients receiving benefits were included in more than 1,000 letters.

5. As of now, no identity theft has occurred.
