5 things to know about medical errors since ‘To Err is Human’

Despite recent efforts to improve quality in healthcare and reduce medical errors, there are still several instances of medical errors at healthcare facilities across the country, according to a report on Philly.com.

Here are five key highlights:

1. Since the 1999 Institutes of Medicine report “To Err is Human,” annual adverse events have hit 100,000 to 400,000 per year.

2. Central-line catheter infections have been reduced by 80 percent across the country and hit zero in some hospitals.

3. There are 120 adverse events per 100,000 hospital admissions each year.

4. There are around 4,000 surgical near-misses or adverse events, including wrong-site surgery.

5. A survey of more than 20,000 bedside nurses found most wouldn’t recommend their hospital to family or friends, and most believed their management team didn’t respond to patient safety issues and were uncomfortable reporting safety violations.
