Margate, N.J.-based Horizon Eye Care Laser and Surgery Center provided notice of a data security event.
On or about June 8, Horizon Eye Care identified suspicious activity within its systems and launched an investigation to determine the scope of the breach, according to a recent notice published by Horizon Eye Care.
Patient information that may have been impacted includes names, demographic information, treatment information and health insurance information.
The organization’s investigation is still ongoing, and can not identify the specific individuals and information that may have been impacted, the notice said.
Horizon Eye Care Laser and Surgery Center has six locations across New Jersey, according to its website.
