Northwoods Surgery Center in Virginia, Minn., disclosed a data breach impacting its network, according to a May 8 report from Claim Depot.
An investigation determined that an unauthorized party accessed and potentially viewed files on the organization’s computer network between July 11 and Sept. 8, 2025.
Information that may have been impacted includes patient names, addresses, dates of birth, health insurance information, patient medical record numbers, medication information, diagnosis and treatment information and medical claims or billing information.
According to a notice filed with the U.S. Department of Health and Human Services Office for Civil Rights, 5385 individuals were affected in the breach.
