1.    ENT — Tympanostomy (requiring insertion of ventilating tube), general anesthesia (CPT 69436) — 48 min.
2.    Pain management — Injection, single, not including neurolytic substances, with or without contrast (for either localization or epidurography), of diagnostic or therapeutic substance(s), epidural or subarachnoid; lumbar, sacral (caudal) (CPT 62311) — 57 min.
3.    Ophthalmology — Extracapsular cataract removal with insertion of intraocular lens prosthesis (1 stage procedure), manual or mechanical technique (e.g., irrigation and aspiration or phacoemulsification) (CPT 66984) — 99 min.
4.    Urology — Circumcision, surgical excision other than clamp, device, or dorsal slit; older than 28 days of age (CPT 54161) — 102 min.
