ASCs worked 38% of their oldest accounts in 2025, compared with 92% of accounts less than 30 days old, according to HST Pathways’ 2026 State of the ASC Industry Report.

The report draws on data from 682 surgery centers in 45 states.

Here are five things to know:

1. Teams worked 92% of accounts in the 0-30 day bucket in 2025.

2. That share fell to 50% for accounts aged 31 to 120 days and 38% for older accounts.

3. Some accounts sat with no activity for up to 87 days.

4. Net collection rates rose from 81% in 2024 to 87% in 2025, but some specialties still lag. Pain management was at 54% and spine at 73%. Centers with 10 to 14 ORs were at 73%.

5. Payers are using more automation and AI to scrutinize and deny cases, the report said. It recommends a more technology-driven approach to revenue cycle management so older balances get addressed on time.

At the Becker’s 32nd Annual Meeting: The Business and Operations of ASCs, taking place October 29-31 in Chicago, ASC leaders, surgeons and healthcare executives will explore strategies to drive growth, enhance operational performance, navigate reimbursement challenges and prepare for the future of ambulatory surgery.
