1. Large hospitals are more likely identified as poor performers on several measures that had low complication rates which increased the likelihood of receiving value-based penalties.

2. Small hospitals scored lower on quality measures with high probabilities of complications.

3. Researchers suggested the program penalized hospitals at both ends of the spectrum because of the penalty methodology.

4. Although a large hospital may have a low overall complication rate, because of the quantity of cases and the amount of risk they're admitting they're in the lower quartiles for performance per CMS guidelines.
