According to HCPF officials, the names of 3,590 medical aid applicants were on the lost disk , though the data did not include birth dates, Social Security numbers or other personal information that could put residents at risk of identity theft.
The data did include the addresses and state ID numbers for the applicants. The disk was lost in transit between two state agencies. Last summer, the department sent apology letters to 10,000 Colorado residents after a computer hard disk was misplaced from equipment returned as surplus.
