Collaborative Health, Humana ramp up value-based care: 5 things to know

Collaborative Health Partners and Humana are collaborating to provide value-based healthcare to Humana’s 4,000 Medicare Advantage members in Lynchburg, Va., and Central Virginia.

Here are five things to know:

1. The collaboration is designed to offer personalized healthcare in a coordinated fashion.

2. The two organizations plan to put primary care physicians in the center of the healthcare system.

3. The effort will combine Humana’s population health abilities, like predictive analytics and chronics disease management, with Collaborative Health Partners’ integrated healthcare network.

4. Collaborative Health Partners involves 100 physicians.

5. Humana has 1.6 million Medicare Advantage members and 200,000 commercial members. About 44,000 primary care physicians treat these members.

“The primary care physician is the key to creating a more effective healthcare system,” said Neil Steffens, Humana Medicare products president. “Value-based care creates a framework that allows primary care physicians to spend more time with patients…”
