CMS Releases Updated Medicare Enrollment Forms

The U.S. Office of Management and Budget has approved changes to the Medicare Provider-Supplier Enrollment Applications and the CMS-8550 application form for ordering and referring items or services to Medicare beneficiaries, according to an AAPC release.

Providers and suppliers enrolling for the sole purpose of ordering and referring items and services to Medicare beneficiaries must use the new CMS-8550 form immediately. Providers and suppliers using the other CMS-855 forms to enroll in Medicare are encouraged to begin using the revised forms, though they may continue to use the old forms through Oct. 2011.
