Long Beach, Calif.-based Golden State Orthopedics & Spine provided notice of a data security incident.
On July 2, Golden State Orthopedics & Spine detected unusual activity within its network and engaged third-party forensic experts to investigate, according to an Aug. 27 notice.
The investigation determined a limited amount of protected health information may have been subject to unauthorized access, including names in combination with addresses, dates of birth, Social Security numbers, health insurance information and medical diagnosis information.
The practice said it has no evidence of actual misuse of any information as a result of the incident. It has implemented additional security measures and is reviewing potentially impacted files to identify the individuals whose information may have been involved.
