1. 0 to 30 days: 53.4 percent
2. 31 to 60 days: 17.2 percent
3. 61 to 90 days: 8.2 percent
4. 91 to 120 days: 5.4 percent
5. Over 120 days: 15.9 percent

In ASCs with fewer than 3,000 cases
6. 0 to 30 days: 40.7 percent
7. 31 to 60 days: 18.7 percent
8. 61 to 90 days: 9.6 percent
9. 91 to 120 days: 5.2 percent
10. Over 120 days: 16.3 percent

In ASCs with at least 6,000 cases
11. 0 to 30 days: 58.2 percent
12. 31 to 60 days: 16.6 percent
13. 61 to 90 days: 7.4 percent
14. 91 to 120 days: 4.8 percent
