The Joint Commission revised its national patient safety goal for anticoagulant therapy in ambulatory care settings.

The key details to know:

1. The revisions address patient and family education, patient monitoring, approved protocols and new practices for Heparin and Warfarin use.

2. The guidelines take effect July 1. and the goal doesn't apply to episodic and occupational health providers.
