Staff Education Tool: Patient Observation of Healthcare Worker Hand Hygiene Tool

The Johns Hopkins Center for Innovation in Quality Patient Care provides a free, downloadable and adaptable card for patients to fill out when observing whether their healthcare providers are performing proper hand hygiene.

 

The two-sided care asks the patient to note whether their physician or other healthcare worker performed hand hygiene and then to submit the card to the organization.

 

On the front side of the card is where patients note whether hand hygiene was performed. The back side of card explains what patients should do with the card and why they are being asked to fill out the card.

 

Johns Hopkins provides an image of the card used at Johns Hopkins and then provides a template organizations can use to customize their own card.
