The Institute for Safe Medication Practices has published a free, downloadable two-page list of error-prone abbreviations, symbols and dose designations.
The problematic abbreviations, symbols and dose designations found in the table were identified through the ISMP Medication Error Reporting Program as being frequently misinterpreted and involved in harmful medication errors.
