Inspectors from the state’s Department of Human Services investigated the hospital after a patient fell from an 11th story window to her death. In addition to lack of staffing, the inspectors cited the hospital for not keeping 21 hospital room windows locked.

The hospital, which since implemented corrective actions, could lose its Medicare status if any citations during the follow-up survey are found or if the hospital fails to remedy those newfound deficiencies.
