Hartford HealthCare and American Ambulance received Connecticut’s first Mobile Integrated Health program designation.
The program delivers medical services outside traditional healthcare settings, most often in patients’ homes, according to a Sept. 8 news release. Specially trained mobile clinicians, including paramedics, nurses and other healthcare professionals, provide the services.
The program is designed to improve access for individuals with complex medical conditions, chronic illnesses, mobility challenges or other barriers to receiving timely healthcare services. Services include in-home assessments, wellness checks, care coordination and referrals to medical and community resources.
The program’s goals include reducing unnecessary emergency department visits and preventable hospital readmissions while supporting existing healthcare providers, hospitals and community organizations.
