The plastic surgery center consented to a $4,000 fine, a reprimand and two years’ probation on May 24, after investigators discovered problems that put public health at risk, according to the report. Dr. Singer said 90 percent of the deficiencies were related to paperwork, and since the investigation, the surgery center has been in compliance.
The 23 violations included failure to provide documentation that meetings of the facility’s governing body occurred at least four times a year and failure to provide current physician credentialing information. The report also said the facility’s owners did not provide documentation that showed all nursing personnel were CPR-certified.
