Here are six notes:
1. Dr. Theodore initially said that he had taken an anti-nausea drug, but later admitted to using Demerol.
2. Dr. Theodore practices with Blue Mountain Anesthesia Associates in Carlisle. Carlisle Regional Medical Center contracts with the anesthesia practice.
3. The state Attorney General’s Office charged Dr. Theodore with “the administration of a controlled substance and acquiring or possession of controlled substance.”
4. Additional charges by the state AG against Dr. Theodore include “misdemeanor counts of refusal to keep records, theft by unlawful taking and possession of drugs with intent to manufacture or deliver.”
5. Dr. Theodore waived his right to a preliminary hearing and is scheduled for a formal arraignment on July 21.
6. Dr. Theodore has not provided care at the hospital since the incident, according to a hospital spokeswoman.
