Legacy Health to shutter outpatient clinics as financial pressures mount

Portland, Ore.-based Legacy Health plans to close several clinics and programs across Oregon and Washington amid ongoing financial pressures, the system said in an Oct. 19 statement shared with Becker’s.

Here are seven things to know:

1. In the statement, Legacy cited rising labor and supply costs, stagnant reimbursement rates and “broader uncertainties” in healthcare as major financial challenges. The system said it faces particular strain because about two-thirds of its patients are covered by Medicare or Medicaid,  programs that often reimburse below the cost of care.

2. According to Legacy Health, new mandates in Oregon and Washington, including required nurse-to-patient staffing ratios and expanded presumptive eligibility for financial assistance, have further increased costs and reduced revenue.

3. Despite more than 200 position reductions and other operational changes over the past three years, Legacy anticipates losing about $38 million in the current fiscal year. The organization also warned that the federal HR1 proposal could further reduce Medicaid and safety-net funding.

4. Here are the several specialty programs that will end in 2026.

  • Legacy Devers Eye Institute at Emanuel Medical Center in Portland will close Jan. 9.
  • Emanuel Outpatient Neuro-Rehabilitation will end in early 2026.
  • Good Samaritan Medical Center’s Cardiac and Pulmonary Rehabilitation program will end Jan. 9.
  • Emanuel Outpatient Rehabilitation will narrow its focus to Legacy Medical Group and trauma patient referrals beginning in 2026.

5. Additionally, Legacy’s Salmon Creek Pain Clinic in Vancouver, Wash., will close March 20, 2026.

6. All Washington urgent care clinics will close. Beginning Nov. 15, Legacy-GoHealth Urgent Care will close its Washington locations in Camas, Cascade Park, Hazel Dell and Salmon Creek. 

7. Legacy will close its Pearl District and Williams urgent care clinics in Oregon. However, eight Legacy-GoHealth Urgent Care centers across Oregon will remain fully open.
