Nashville, Tenn.-based Covenant Surgical Partners, an operator of single and limited specialty ASCs, experienced a data breach affecting 88,609 individuals, according to the U.S. Department of Health and Human Services Office for Civil Rights.
The data breach was reported May 28 and was a hacking/IT incident of the company’s network server.
Law firm Levi & Korsinsky launched an investigation into the breach. According to a June 9 news release, information that may have been affected includes names, addresses and contact information, dates of birth, medical records, insurance information and payment information.
