Here’s what you should know:
1. The group will select projects that show potential to overcome patient-provider barriers by establishing easily adoptable best practices.
2. The group will select two projects by Nov. 22. The projects will begin receiving funding in December 2017 and will be fully funded from 2018 through 2019.
3. Interested parties “are encouraged to design programs or initiatives that explore or address gaps in communication in areas ranging from lifestyle and social issues to challenges related to determining treatment options.”
4. The group is accepting proposals from Sept. 11 to Oct. 12.
