1. SonarMD is a web-based application delivering care management algorithms to help physicians monitor and respond to patients.
2. The enhancements include:
- An optional comment window
- An optional question window
- A detailed dashboard
- A manual survey page
- An additional page containing a patient’s Supervisit report
- A “Watch List” tool
- A communications tab
3. Lawrence Kosinski, MD, MBA, a gastroenterologist and managing partner of the Illinois Gastroenterology Group, developed the SonarMD platform.
4. Many gastroenterology practices across the country have adopted SonarMD since its launch in 2015, and more than 100 physicians and nurse care managers are using the platform to manage care for patients with Crohn’s disease. Currently, four more practices are in the onboarding process.
5. SonarMD and Blue Cross and Blue Shield formed an Intensive Medical Home agreement.
