Joint Commission: New tools for medical error prevention

The Joint Commission has several tools to help prevent medical errors, including the “Speak Up” campaign.

The organization released a video on “patient identifiers” titled “Speak Up: Right ID, Right Care.” The video addresses appropriately identifying patients to make sure they receive the right care at the right time using:

• Birth dates
• Wristband checks
• Phone numbers

A separate video titled “Speak Up: Prevent Errors in Your Care” focuses on medical error prevention:

• Checking to make sure care is appropriate
• Asking for clarification questions
• Encouraging patients to speak up
