5 key notes on Joint Commission’s 1st Integrated Care Certification

Parrish Medical Center in Titusville, Fla., became the first facility to receive The Joint Commission’s Hospital Accreditation Program Integrated Care Certification.

Here are five key notes:

1. The Joint Commission began offering the integrated care certification in July of last year for hospitals and ambulatory care facilities with excellent integration in information sharing, care transition, hand-off communications and other key activities.

2. The program’s goal is to engage providers and patients throughout the care cycle and integrate care across the continuum for better patient outcomes.

3. Eligible facilities have at least one component of the healthcare system, including ambulatory care or hospital services, accredited by The Joint Commission’s three year certification. The organization must then undergo a special integrated care review separate from the accreditation process.

4. Parrish Medical Center is a 210 bed, not-for-profit medical center that became part of the Mayo Clinic Care Network in 2014.

5. The hospital undertook the special voluntary certification process to show the community-based network of hospitals, physicians and outpatient services work together for better patient care and reduced costs.
