•    Top two physicians: 39 percent
•    Top five physicians: 68 percent
•    Top ten physicians: 91 percent

Three to four ORs
•    Top two physicians: 29 percent
•    Top five physicians: 53 percent
•    Top ten physicians: 75 percent

More than four ORs
•    Top two physicians: 22 percent
•    Top five physicians: 43 percent
•    Top ten physicians:62 percent
