•    Employee salary and wages: 22.4 percent
•    Taxes and benefits: 4.3 percent
•    Occupancy costs: 7.7 percent
•    Medical and surgical: 19.4 percent
•    Other medical costs: 0.8 percent
•    Insurance: 0.7 percent
•    General and administrative: 22.8 percent
•    Total operating expenses: 66.5 percent

Three to four ORs:
•    Employee salary and wages: 22.3 percent
•    Taxes and benefits: 5.0 percent
•    Occupancy costs: 6.8 percent
•    Medical and surgical: 21.3 percent
•    Other medical costs: 1.0 percent
•    Insurance: 0.7 percent
